## The handoff is where your part of the story stops

EMS work has a strange shape. You meet people at the worst moment of their lives, do everything you can in a cramped space with limited time, hand them off at the emergency department, and then clear for the next call. You rarely find out what happened. Did the cardiac arrest make it? Did the kid from the rollover wake up? The story just stops.

## Volume without a pause button

Many EMS systems run lean. Back-to-back calls, long shifts, overtime to cover gaps, meals in the truck. There is no time between a death on scene and the next call to feel anything, so the feeling gets stored for later. Later often never comes. After enough of that, some people go numb, some get angry, and some find that a minor call suddenly hits harder than it should.

## The calls that stay

Every medic has a few. Pediatric calls often top the list: a child who is not breathing, an infant death, abuse. Deaths on scene, especially when family is present. Suicides. Calls that felt too much like your own family. These can come back as images, smells, sounds or a physical reaction in a similar setting. ## Running on empty

Fatigue in EMS is not just tiredness. It is chronic short sleep, night shifts, and a body that is always half-waiting for the next tone. Exhaustion makes everything else harder: patience, mood, decision-making, relationships. ## How the work starts

Zandria Hooks, LCSW, is EMDR trained and CPT certified and uses CBT. She is also prior active duty and serves in the National Guard as a mental health provider, so a culture built on calm under pressure and dark humor does not need translating. The first sessions focus on what you want help with and on getting enough stability to do deeper work. You set the pace.

Sessions are telehealth only, 50 minutes, with evenings available, for adults physically located in Texas, Maryland or Massachusetts at the time of each session.
